My trip to the United States was primarily for customer meetings, but before returning to Germany, I seized the opportunity for this insightful observation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I rode with <strong>two Emergency Medical Technicians (EMTs)<\/strong> from New York\u2019s first EMS station at <strong>Bellevue Hospital<\/strong>, experiencing firsthand the challenges and operations of the city&#8217;s pre-hospital emergency care system.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As I sit in <strong>EMS Unit 1397<\/strong> at NYU Langone Health, awaiting the next call after delivering a patient, I reflect on the professionalism and dedication of the team I joined. As someone still active in emergency medicine in Germany, I was eager to compare their handling of medical emergencies, their workflow, and the equipment and technologies they use.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Key Differences Between U.S. and German EMS<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>1. Mobile EMS Units<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Unlike in Germany, where ambulances return to base between calls, EMS units in New York are assigned to <strong>specific areas (approximately four blocks)<\/strong>. Their vehicles serve as their workspace and temporary home for <strong>8, 12, or 16 hours<\/strong>, depending on the shift schedule.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>2. Siren Fatigue<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Traffic congestion in New York significantly impacts EMS response times. Even with <strong>lights and sirens<\/strong>, many drivers fail to yield, likely due to what I interpret as <strong>siren fatigue<\/strong>. For one emergency transport, we covered <strong>just 5 kilometers in 18 minutes<\/strong>, a delay that could be critical in life-threatening situations.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>3. Minimal Equipment on BLS Ambulances<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">BLS ambulances in the U.S. carry <strong>less equipment<\/strong> than standard German ambulances (Rettungswagen, RTW). Notably, they <strong>lack ECG monitors<\/strong>, <strong>cannot administer IV lines<\/strong>, and <strong>do not perform endotracheal intubation<\/strong>. Additionally, <strong>ventilators are absent<\/strong> on board. Interestingly, each EMT carries a <strong>personal medical bag<\/strong>, individually stocked according to preference and experience.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I didn\u2019t have the chance to work with an <strong>ALS (Advanced Life Support) unit<\/strong>, but I learned that they follow a <strong>significantly higher equipment standard<\/strong>, allowing paramedics to <strong>administer medication, intubate, and ventilate<\/strong> patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>4. Fully Digital Medical Documentation<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patient records in the U.S. EMS system are <strong>completely digital<\/strong>, a feature that impressed me. EMTs can <strong>retrieve medical histories and allergy information<\/strong> for previous patients. In one case, this was potentially life-saving, as the patient had an <strong>aspirin allergy<\/strong>, a critical consideration in treating <strong>Acute Coronary Syndrome (ACS)<\/strong>. This type of seamless digital integration is something <strong>Germany could greatly benefit from<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>5. On-Board Teleconsultation<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Unlike in Germany, U.S. EMS systems <strong>are not physician-based<\/strong>. Instead, EMTs and paramedics communicate <strong>directly with hospital physicians<\/strong> via <strong>bi-directional teleconsultation<\/strong>, receiving remote medical guidance when needed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>6. Common Medical Conditions<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">New York EMS deals with <strong>a high number of drug-related and psychiatric emergencies<\/strong>. <strong>Gunshot wounds and stabbing injuries<\/strong>, while less common in downtown Manhattan, remain <strong>alarmingly frequent<\/strong>, much more than in Germany. Many incidents are tied to <strong>domestic violence<\/strong>, highlighting ongoing social challenges.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>7. Heavy Police Presence<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Police presence in public emergencies is noticeably greater in New York compared to Germany. Even in <strong>routine cases<\/strong>, such as a <strong>suspected stroke in a subway station<\/strong>, <strong>four police officers<\/strong> secured the EMS team while treating the patient.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Due to the <strong>perceived higher risk level<\/strong> during EMS operations in New York, the <strong>emergency dispatch center<\/strong> proactively checks the <strong>safety and operational status<\/strong> at predefined intervals using <strong>digital radio communication during missions<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>8. Ambulance Interior and Working Space<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">New York EMS vehicles are <strong>spacious<\/strong>, accommodating <strong>up to four people plus a stretchered patient<\/strong>. However, the <strong>standing height<\/strong> inside the vehicle is <strong>significantly lower<\/strong> than in German ambulances (approximately <strong>1.70 meters<\/strong>). This could make <strong>trauma patient care<\/strong> more challenging, as the <strong>stretcher height is fixed<\/strong> and cannot be adjusted.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>9. Training Duration<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Becoming an <strong>Emergency Medical Technician (EMT)<\/strong> in the U.S. takes approximately <strong>three months<\/strong>, whereas <strong>Paramedic training<\/strong> requires <strong>around nine months<\/strong>. This is shorter than Germany\u2019s training requirements, where EMTs undergo more extensive education.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Final Thoughts<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">My time with <strong>New York EMS<\/strong> was deeply inspiring and insightful. I am grateful to the team for their openness and willingness to answer countless questions.
